Inpatient Service

The inpatient neurorehabilitation programme is based at Queen Mary’s Hospital, Roehampton. Patients are initially assessed at our multidisciplinary team assessment clinic at which point we identify their suitability for inpatient rehabilitation, review their history and diagnosis, and identify clear therapy goals.
We have 8 beds in our neuropsychiatry inpatient programme at Queen Mary’s.

• 4 Beds are for patients with acquired brain injury / degenerative conditions.
• 4 Beds are for patients with FND.

The treatment approach is multidisciplinary and professionals include:

Physiotherapist, Occupational therapists, Neuropsychologists, Speech and Language therapists, Specialist Nursing, Neuropsychiatry

In addition to these disciplines, we have regular input from rehabilitation, urology and neurology consultants.

A combination of the above therapies is tailored to each patient’s needs. This holistic approach seeks to treat every patient using the bio-psycho-social model.  Therapy goals are different for each person and are identified by patients. Often these include improving mobility, increasing independence with specific tasks, managing pain, as well improving fatigue and mood management.
The average length of stay on the ward is between 6-12 weeks. Prior to discharge an on-going management plan is formulated to support the transition back into the community. This may involve referral to additional services such as community rehabilitation and outpatient follow up.

Inpatient Services

We currently provide different pathways for treatment, dependant on the outcome of the assessment:

·         Inpatient Treatment: 4 week programme with treatment sessions taking place Monday to Friday, with weekend home-leave.

Patients referred to our service have complex-needs and may have a combination of symptoms including but not exclusively motor-disorder.  We are not a Pain or Fatigue Management Service.

Our inpatient and outpatient programmes offer similar input of Cognitive Behavioural Psychotherapy (CBT), Specialist nursing (Mental Health and General), Physiotherapy and Occupational Therapy. Patients not willing to consider psychological therapies as a part of their treatment will not be accepted for our programmes and are advised to seek community therapy-services referrals via their GP.

Inpatient Services

The NICPM is an eight bed specialist inpatient unit based at Leeds General Infirmary. It is a unique service which has a history over many years of delivering services within Leeds and West Yorkshire, and since 2009 has been successfully treating patients from across the UK.

Our team specialise in helping people with the following types of problems:

• Severe and complex medically unexplained symptoms and illness
• Psychological difficulties affecting the management of long-term physical health conditions (physical / psychological comorbidities) at a serious level of severity
• Severe chronic fatigue syndrome (CFS/ME) – we provide the inpatient care for the Leeds and West Yorkshire CFS/ME service

Approximately half of our beds are commissioned for Leeds residents and half for people from across the UK. For Leeds residents we also provide a service for people with mixed physical and psychological difficulties which may be complicated but at a more moderate or lower level of severity.

Our aim is to help people make significant improvements to their health and quality of life. Our clinical outcomes are often very good and patient feedback is very positive as you will see in our  annual reports at the end of this page.

Referral Process

Referrals into our service are made by your GP or another health professional involved in your care. They will need to make a referral to us that includes your recent blood tests and other essential information we require.

If you live outside Leeds, we will need to request funding from the clinical commissioning group (CCG) local to where you live following your referral from your GP.

Outpatient Services

Community Psychological Medicine in Berkshire provides specialist outpatient clinics for patients with FND. The clinic accepts referrals from GPs and other healthcare professionals.

Referral Process

Following diagnosis patients can be referred to the FND Psychology Service and/or Rehabilitation Services (Physiotherapy, Occupational Therapy and Speech and Language Therapy). Please see FND psychology referral criteria below:

Inclusion Criteria – We are open to referrals for assessment when a community patient:

• Is 18 years old or above
• Has one or more functional neurological symptoms
• Has been diagnosed with functional neurological disorder (FND) by a Neurologist
• Have completed all diagnostics
• Have been informed of the functional diagnosis
• Has agreed to a referral to the FND team to see a Psychologist
• Has a GP within Exeter, East Devon and Mid Devon Localities

Exclusion Criteria – We are unable to provide an assessment and intervention service for patients:

• Who are awaiting test results
• Who are pursuing further diagnostics
• With a primary unmanaged mental health presentation:
  • Presenting with dissociation or personality changes not characterised by apparent seizure activity or functional symptoms
  • Whose functional symptoms occur in the context of an unmanaged mental health condition e.g. Emotionally Unstable Personality Disorder, PTSD, or Borderline Personality Disorder.
• Who are currently excessively using alcohol or illicit substances
• Who are unwilling for us to liaise with health colleagues across the RD&E and DPT where it is appropriate.

In regard to patients subject to exclusion criteria, we are happy to provide an advice and consultation service for clinicians working with those patients in our catchment area. The aim is to support colleagues to practice in FND-informed ways and to enable a realistic care-pathway to be established to enable recovery or better management of symptoms.

Outpatient Services

They promote independence whilst maximising safety and managing risk.  They aim to minimise impairment, while enhancing activity, participation, wellbeing and quality of life. They strive to be responsive and accessible in providing high quality integrated client-centred services.

Outpatient Services

ICNRT are a multi-disciplinary team offering physiotherapy, Occupational Therapy, Psychology, Speech and Language Therapy and rehab support worker input and are in the process of formalising an FND pathway.

Fous is on:

• Increasing clients independence at home and within the community
• Improving clients wellbeing
• Improving clients quality of life

They provide one-to-one individual therapy, within the community in Client’s home and the Outlook Centre.  ICNRT also provide a number of specialised therapy groups, such as exercises group for balance and a wellbeing group focused on supporting people with long-term health conditions.

Referral Process

Referrals to the Walton Centre are made from primary or secondary care. If you have neurological symptoms which you think might be suggestive of FND, referral to any of the Walton Centre Neurologists at the most convenient hospital can be made by your GP or other medical team. You will then be seen in an outpatient clinic and assessed and a plan will be made as to whether any tests are required. Once an established diagnosis is made, your neurologist will discuss the options with you. There is a monthly Multidisciplinary team meeting where the Neurologist can discuss patients they have seen if the diagnosis or best course of action is not clear. The team will offer advice in order to ensure that they receive the most appropriate and helpful support within the available services.

Outpatient Services

The predominant therapeutic approach is individual Psychodynamic Interpersonal Therapy, although we also offer individual Cognitive Behavioural Therapy where appropriate.

Typical referrals include:

people with symptoms that appear to be due to neurological illness (e.g., non-epileptic seizures, paralysis, movement difficulties, sensory disturbance) but which have been diagnosed as functional by a neurologist following appropriate investigation;

Outpatient Services

We are a specialist team of psychological therapists who work with individuals to understand and manage the symptoms of NEAD.
We offer group information sessions as well as individual psychological therapy and training for other healthcare professionals.
Each person’s experience of NEAD is different, which means that the support that we provide will be guided by the specific needs of each individual.

Patients are required to have received a diagnosis of non-epileptic attack disorder and have been referred to the service by a Salford Royal NHS Foundation Trust Consultant.

Outpatient Services

At present we have part of the time of a Consultant Neuropsychiatrist, who offers assessment/diagnosis, psychiatric treatments and referral, and part of the time of a Consultant Clinical Neuropsychologist and a Clinical Psychologist who offer assessment/diagnosis and MBCT (mindfulness-based cognitive therapy), and we have the possibility of referral to our local rehab Physiotherapy team. As appropriate, referrals can also be made to out-of-county specialist FND services.

Referral Process

We accept referrals from Consultant Neurologists working in Sheffield, Barnsley, Rotherham, Doncaster and Bassetlaw Hospitals. The referral route for diagnosis by a Consultant Neurologist is usually through your GP, and or other hospital doctors following admission or emergency treatment.

After someone is referred to our service, they are initially contacted by post and asked to fill in a questionnaire. After questionnaires have been returned and received by the team (we provide prepaid envelopes), patients are added to our waiting list. When patients reach the top of the waiting list they are contacted by telephone or letter to make an appointment to see a therapist.
